This document contains the end matter published in Volume II, which consists of the Endnotes, Bibliography, Index, Acknowledgments and About the Author. It is primarily intended for readers who only have Volume I and would like to make that book the equivalent of a self-standing book, although those who have Volume II may want to have this information in a printable format.
